What is Alkaline Phosphatase ?

The Serum Alkaline Phosphatase (ALP) test measures the level of alkaline phosphatase in the blood. ALP is an enzyme found in various tissues throughout the body, including the liver, bones, kidneys, and bile ducts. Elevated levels of ALP can indicate liver disease, bone disorders, or other health conditions. Clinicians may order a Serum Alkaline Phosphatase test to: Assess liver function, as high ALP levels can signal liver conditions such as cholestasis, liver tumors, or bile duct obstructions. Evaluate bone health, since elevated ALP can also indicate bone diseases like Paget's disease, osteomalacia, or bone cancers. Monitor kidney function, as ALP levels can rise in certain kidney conditions.

Here’s what an ideal Alkaline Phosphatase report format should include:

Header Information:

  • Patient Details: Full name, Age, Gender, and ID.
  • TAT information: Timestamp for both sample collection and report generation.
  • Doctor's Information: Name of the referral doctor, if applicable.

Test Results Section:

Physiologic Basis:
Alkaline phosphatases are primarily found in liver, bone, intestines, kidney, and placenta. Test is used to detect liver disease and bone disorders.

Interpretation:
Increased in
: Obstructive hepatobiliary disease, bone disease (physiologic bone growth, Paget disease, osteomalacia, osteogenic sarcoma, bone metastases), hyperparathyroidism, rickets, benign familial hyperphosphatasemia, pregnancy (third trimester), GI disease (perforated ulcer or bowel infarct), hepatotoxic drugs.
Decreased in: Hypophosphatasia.

Comment:
Alkaline phosphatase performs well in measuring the extent of bone metastases in prostate cancer. Alkaline phosphatase isoenzyme separation by electrophoresis or differential heat inactivation is unreliable. Use γ-glutamyl transpeptidase, which increases in hepatobiliary disease but not in bone disease, to infer the origin of increased alkaline phosphatase.
