What is Sodium ?

The Serum Sodium test measures the level of sodium in the blood. Sodium is an essential electrolyte that helps regulate fluid balance, nerve function, and muscle function. The body tightly controls sodium levels, and abnormalities can indicate dehydration, kidney problems, or hormonal imbalances. Clinicians may order a Serum Sodium test to: Assess hydration status, as low sodium (hyponatremia) can indicate dehydration or overhydration, while high sodium (hypernatremia) may suggest dehydration or kidney dysfunction. Evaluate kidney and adrenal function, as abnormal sodium levels can be a sign of kidney disease, adrenal gland disorders, or other metabolic issues. Monitor patients on specific medications, such as diuretics, which can affect sodium levels and fluid balance.

Sodium Report Format: Breakdown

Here’s what an ideal Sodium report format should include:

Header Information:

  • Patient Details: Full name, Age, Gender, and ID.
  • TAT information: Timestamp for both sample collection and report generation.
  • Doctor's Information: Name of the referral doctor, if applicable.

Test Results Section:

  • Patient's results

    As obvious as it is, a test report should definitely have the patient's test result.

  • Result's Unit

    The unit of the test result must be mentioned correctly in the report. Sodium test is generally reported in “mmol/L”

  • Sodium Normal Value / Reference Range

    The report must have the normal Sodium range. It can differ slightly based on the reagents used by the lab and other internal factors, but common Sodium ranges are:

    For any gender, 0 to 100 year of age: 136 - 146

Physiologic Basis
Sodium is the predominant extracellular cation. The serum sodium level is primarily determined by the volume status of the individual. Hyponatremia can be divided into hypovolemia, euvolemia, and hypervolemia categories.

Interpretation
Increased in: Dehydration (excessive sweating, severe vomiting, or diarrhea), polyuria (diabetes mellitus, diabetes insipidus), hyperaldosteronism, inadequate water intake (coma, hypothalamic disease).
Drugs: steroids, licorice, oral contraceptives.
Decreased in: CHF, cirrhosis, vomiting, diarrhea, exercise, excessive sweating (with replacement of water but not salt, eg, marathon running), salt-losing nephropathy, adrenal insufficiency, nephrotic syndrome, water intoxication, syndrome of inappropriate antidiuretic hormone (SIADH), AIDS.
Drugs: thiazides, diuretics, ACE inhibitors, chlorpropamide, carbamazepine, antidepressants (SSRI), antipsychotics.

Comments
Hyponatremia in a normovolemic patient with urine osmolality higher than serum (or plasma) osmolality suggests the possibility of SIADH, myxedema, hypopituitarism, or reset osmostat. Treatment of disorders of sodium balance relies on clinical assessment of the patient’s extracellular fluid volume rather than the serum sodium.
